Research & privacy.
Survey response collection is not open. The questions can be reviewed, but the preview cannot save your answers. A working study contact address will be published before collection opens.
Who is conducting the study?
Care Operations Lab is a private research and software-development project led by Sean J. Vazquez in Massachusetts. It is not affiliated with the Massachusetts Executive Office of Aging & Independence, another regulator, or a participating employer.
Purpose and use
We are studying assisted living operational work, existing tools, unmet needs and purchasing considerations. Findings will guide potential software products. Aggregate findings may also be published in free summaries or commercially licensed research reports. We do not sell identifiable individual responses or contact lists.
The study is not a regulatory inspection, clinical assessment or facility-rating exercise. Confidential research responses will not be repurposed into identifiable facility ratings or endorsements.
What we ask for
Optional information includes role, residence town and name, parent operator, broad operating profile, operational experience and purchasing considerations. A response scope and participation consent are required when collection opens. Optional work email and contact permissions are kept separately from the answer record and may be linked using a response reference.
Do not provide resident or employee identifiers, health information, incident narratives, confidential agreements or employer documents. Share only information you are authorized to share. The study is confidential, not anonymous: your residence, contact information or invitation context may identify you.
Choice and contact permissions
You can skip substantive questions or leave without submitting. Separate, unchecked choices cover the research summary, research interviews, pilot discussions, report marketing and software marketing. A pilot discussion does not commit you to a purchase. A research permission does not enroll you in marketing.
You may use email preferences to stop future invitations and marketing. Requests do not require an account. After collection opens, you may request withdrawal of an identifiable response by contacting the study team with your response reference. Findings already incorporated into non-identifiable aggregate reports may not be removable.
Storage and access
Submitted responses will be stored on our Hostinger hosting service. Response content and contact details are encrypted by the application before database storage. Access is restricted to authorized project administration. Hostinger processes site and service data to provide hosting; its policies also apply to its own processing.
The site uses a necessary session cookie for submission protection. Preview answers are held in page memory, not browser storage, and are not submitted. Hosting systems may retain ordinary request logs, including IP addresses. For abuse prevention, the application uses short-lived, keyed address hashes rather than storing raw IP addresses in its research database. We do not use advertising trackers or email-open tracking pixels on this site.
Retention and reporting
Our planned retention period for raw research responses is up to 12 months after the study closes. Contact details are kept only while needed for the selected purposes; withdrawal ends further contact for those purposes. A minimal suppression record may be retained to honor an opt-out. Technical backups follow the hosting provider’s retention cycle, so removal from backups may take longer than removal from the active database.
We will review free text before reporting and remove identifiers. Reports will state the collection dates, respondent count, recruitment method, coverage limits and relevant missing data. Small groups may be combined or omitted to reduce identification risk. An open voluntary survey does not automatically represent all Massachusetts residences.
Sources and external information
Official Massachusetts residence information is available through the state’s assisted living residence directory. The directory is a source for research planning and does not imply state sponsorship.
Hostinger publishes its privacy policy. This notice will be updated before collection opens with the monitored study contact and confirmed operational details.
